Synopsis : Ippo, jeune prodige de la boxe, a afin atteint son but : le titre de champion de boxe du Japon ! Mais ce n'est pas pour autant que notre jeune héros doit s'endormir sur ses lauriers : il se doit de défendre le symbole de son titre, la ceinture du champion, contre les challengers qui désirent la lui ravir. Et voici que son premier match de défense de titre est planifié, contre un certain Sanada, lui aussi prodige de la boxe mais aussi futur médecin. Face au stress occasionné par son premier match en tant que champion, par le fait que son adversaire pratique une boxe « scientifique », par les facéties intrépides du trio Takamura-Aoki-Kimura, mais aussi parce qu'il semble avoir perdu toute chance de relation avec Kumi, qui l'a quitté en mauvais termes, Ippo réussira-t-il malgré tout à conserver cette ceinture si durement gagnée ?
